You will need to box up your hardware needing repair and attach a completed Form 0530 (can be ordered from Stores, Item#35-05-000500)to the outside of the box. BarCode (if available) must be included on form in Equipment Tag field.

Keep the gold copy for your records. The form incorrectly states you should call the Help Desk before sending equipment. You need to call AVRepair at 336-8700 ext: 1132

Items not needing to be boxed: Big TVs, Overheads or large items that won't fit in box.

If it is hand carried in without a box, someone must hand carry back because AVRepair does not have boxes.
